It’s called a "cover and insert" for the GE icemaker.
It wasn’t likely for any parts house to stock this part when it was available. Now, it’s pretty much impossible. I’m including a list of GE fridge models that this part fits. Your best bet is to try craigslist or something like that and see if someone who is giving away one of these models might want to sell/give away just this part.
